USCISMed Exams

If you have been treated for tuberculosis in the past you will need to bring documentation from the facility where you were treated, including medications used to treat, dates of treatment and length of treatment.

 If you have a record of vaccinations you have received please bring documentation with you to our office.  Not sure what immunizations you need? Check our Immunization Page.

Before your appointment, complete the first page of the I-693 form and email the first page and top of the second page to:pfitzpatrick5@partners.org

Immigration exams are not covered by your insurance plan.  Payment is due in full at the time of the appointment.  We accept MasterCard or Visa and  cash.

If you do not speak English you must have an adult present who can read and write  English.  They will also be required  to sign your final I 693 form.

Pre-visit Checklist

A US Government issued photo Idenification such as a valid US drivers license, or a passport  from your home country are examples of valid ID's.  

If you have mental health issues you need to bring all medical records concerning the mental illness.
