Important Facts about Vaccine Requirements

The following Chart will show what vaccines are needed for each age group

Immunization Requirements

  • Flu vaccine is only required during flu season(October to the end of March)
  • Bring all your vaccine records with you to the appointment.
  • Several immunizations come in series (MMR and Varicella), and even if you have already had a vaccine you may need a booster.
  •  You Do Not need an MMR vaccine if you were born before 1958.
  • To avoid receiving immunizations at our office, call your primary care doctor and ask if you can have titers drawn to prove that you are immune to measles, mumps, rubella and varicella.  You will still need a current (within the past 10 years) tetanus (Td or Tdap) vaccine as well as a flu vaccine during flu season, which can be administered here or at your MD's office
  • Medical documentation cannot be finalized until all vaccine requirements have been met.
  • There is a charge for vaccinations as every person has different requirements depending on documented vaccination records that are provided.
